Print the form (you may need to right click on your mouse to get the print screen) or a new $15 rebate wyb $50 of Venus, Secret, Olay, Pantene, Herbal Essences, Aussie, Head Shoulders, Clairol, Covergirl, Crest 3D White Whitestrips, Puffs, Ivory and Safeguard. Must purchase 10/14/12-12/31/12, so it doesn't start until tomorrow. The $50 purchase is the subtotal before coupons are deducted, so feel free to use all the coupons you want. There will be a small new PG insert tomorrow, plus the PG 9/30 insert is still valid. Send form, original dated cash tapes with items circled and write UPCs on form. Multiple receipts are okay. 

You may want to print the rebate form ASAP.  Sometimes they remove them online before the end of the rebate is up.
